Objectives and Results of Hoodplasty in Dubai ?

 Surgical treatment of girls with masculinized genitalia is gradually moving towards a more conservative approach. 

Central to this development are reports of loss of clitoral sensation and associated impairment of sexual function in women who underwent feminizing genital surgery in childhood. 

During follow-up, clitoral glans exposure is sometimes observed, and although patients may complain about the aesthetics, clitoral discomfort resulting from glans exposure has not been reported. A technique for reconstructing the clitoral hood has been developed and this study presents the preliminary results.

Aims

To present a new technique and preliminary results of clitoral Hoodplasty in Dubai reconstruction to cover the exposed glans penis of the former clitoris in patients with congenital adrenal hyperplasia (CAH).

When examining a patient who desires labiaplasty in our practice, it is very important to observe and see if there is also clitoral excess and to show her as she often does not see it before the operation, but when the labia are gone, the excessive clitoral hood becomes more visible and looks like a penis. 



If there is excess clitoral hood on the horizontal axis and/or excessive clitoral hood coverage, clitoral hoodoplasty is preferable.

 The horseshoe plasty is a continuation of the longitudinal labiaplasty; It is important never to over-resect this horizontal axis as this can result in an x-ray of the clitoral head which is very uncomfortable for the patient.

Patients and Methods

Six patients (mean age 13 years, range 4-21 years) with CAH consulted a physician for clitoral problems after previous clitoral reduction surgery.

 In two out of six patients, the main complaint was clitoral hypersensitivity or even pain.The glans of the clitoris was fully exposed in all patients, who then underwent a double-layer V-Y clitorisplasty to create a natural-looking, flexible clitoral hood composed of an inner and outer layer of foreskin.

Results

Postoperative coverage of the glans penis was complete in 5 patients and partial in 1 patient. Two patients with preoperative pain were asymptomatic; All six patients were satisfied with the postoperative aesthetic results.
